Per Diem Social Worker (LMSW/LCSW) — Home Care Program

All Heart Home Care Agency is seeking a per diem Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to support our NYC Human Resources Administration (HRA) home care program, with a focus on Emergency and Exceptional Home Attendant Services (also known as “Difficult to Serve” or DTS cases).
